En 1969, il auditionne pour Hair, la comédie musicale et décroche un rôle qui le fait jouer à Broadway durant un an. Issu d'une famille de comédiens dédiée au cinéma, c'est tout naturel qu'il s'y frotte dès 1971 dans John McCabe de Robert Altman où il fait de la figuration.
Il y interprète un agent du FBI lancé sur les traces du célèbre serial killer. Lorsque son personnage meurt deux ans après, Keith Carradine rejoint Glenn Close dans la série Damages.
